Who we are

1) Privately held Corporation founded by Physicians that Provides and Manages Health Care Contracts.

2) MCCI utilizes 2 different models, MCCI owned and Operated Medical Centers and MCCI contracted Affiliated Physician Practices.

3) MCCI owned and operated model is utilized mainly in South Florida. MCCI Affiliated model is utilized in Northern Florida and Texas.

4) The affiliated model is comprised of a contract between MCCI, a community physician or group and Humana.

Services Provided

1) Assign a Provider Representative to provide the following services:

• Negotiate and Manage Physician contracts.
• Function as the single point of contact for all on-going contractual and service issues.
• Facilitate provider education; provide training to increase their familiarity and satisfaction with our systems and strategies.
• Provide trend analyses and summaries for decision making.
• Coordinate Member retention and activities
• Transportation for members of contracted affiliated physicians

2) Provide Transportation for Humana Gold Members of contracted affiliated physicians via contracted vendor.

3) Provide Hospital Case Nurses to help facilitate Patient needs post Hospital Discharge:

• Summary of Discharge notes to PCP
• Patient leaves with proper medication list
• Facilitate Patient Appointment with PCP within 72 hrs
• Facilitate Patient referrals (home health etc).
• Facilitates Patient Transportation if needed.

4) Provide access to MCCI Acute Care Program

• Physician has ability to refer Humana Gold Patients that have special needs or are high touch high care patients.
• MCCI identifies high touch and high care patients and coordinates appropriate care with PCP and other Physicians.
• Acute Care nurses are assigned to these patients and develop a telephonic POT with MCCI Director and or PCP.
• Acute Care nurses develop telephonic relationships and monitor conditions, vital and progress and help coordinate immediate care as needed. They report daily to the Medical director.
